Applying for unemployment benefits can be challenging and uncertain, and you need to make sure you are well informed about the various mailings and forms you may receive after you apply. Here's a breakdown of the documents you can expect and some important information about each one:

1. BC9 – Unemployment Insurance Instructions and Appointment Notice:

  • It contains instructions on how and when to claim your benefits.
  • Includes a list of upcoming appointments.

2. BC3C – Notice to Applicant of Benefit Determination:

  • Provides details on the weekly benefit rate and maximum benefit amount.
  • Lists employers you worked for, base weeks worked, and wages during the period used to determine benefits.
  • Review it carefully and file an appeal if there is incorrect or missing information.

3. BC5 – Notice of Invalid Claim:

  • It is sent if you were not employed in New Jersey during the base year or if your claim is determined to be invalid.
  • List the reason for the disability.
  • File an appeal if you don't agree with the decision.

4. Prepaid Debit Card:

  • Sent if you choose not to have profits deposited directly into your bank account.
  • The card arrives in an unmarked envelope 7-10 days after you apply for benefits.
  • Valid for four years; keep it until the expiration date.
  • Reactivating your claim may also trigger a new card issuance.

5. Electronic Adjudication Problem Clarification Questionnaire:

  • Received by email if additional information is needed.

6. BC26B – Notice of Determination:

  • Sent after the fact-finding interview (by phone or email).
  • Indicates whether you were granted or denied benefits.
  • Provide dates of ineligibility.
  • Appeal the decision if necessary.

AT2: Notice of Hearing:

  • Received if you or your employer file an appeal.
  • Tells you about the date, time, and instructions for participating in the hearing.
