In addition to standard submissions, you may need to download and print certain forms for various purposes. Here's a guide to some crucial forms and their meanings:

1. PR94 Unemployment Insurance: Your Rights and Responsibilities:

  • Contains information about eligibility for Unemployment Insurance benefits.
  • Provides instructions on how to apply for benefits, important phone numbers and more.

2. Job Search Record:

  • Use these forms to record your job search activities, including potential employers and contacts.

3. WD175 New Jersey Intake and Initial Evaluation Form:

  • Compulsorily completed before attending the Reemployment and Eligibility Evaluation appointment.

4. BC262.2 Worksheet for Reemployment Services and Eligibility Evaluation (RESEA):

5. BC502 Authorization for Payment of Benefits by Direct Deposit or Debit Card:
