Documents received after applying for unemployment benefits

After applying for unemployment benefits, you will receive several letters from the Department of Labor and Workforce Development. These letters will likely arrive on different days and not all at once. Depending on your claim, you may not receive all of the documents mentioned below. The Department may also attempt to contact you by email.

BC9 – Unemployment Insurance Instructions and Appointment Notice
This form tells you how and when to claim your benefits. It also includes a list of upcoming appointments.

BC3C – Notice to Claimants Regarding Benefit Determination
You should only receive this form if you were employed in New Jersey during the period used to calculate your benefits. This form will show you the amount of money you can receive weekly (weekly benefit rate) and the total amount you may be entitled to collect during the one-year period in which your claim is active (maximum benefit amount).
This notice will also include the employers you worked for and the wages you received during the period considered to determine your benefits. Review all of this information carefully. If you notice anything wrong or missing, file an appeal.

BC5 – Notice of Invalid Claim
You should only receive this form if you were unemployed in New Jersey during the base period or if your claim is deemed invalid for other reasons, such as if you were a corporate officer or were not authorized to work by U.S. Citizenship and Immigration Services (USCIS). This notice will state the reason your unemployment claim is invalid. If you disagree with the decision, file an appeal.

Prepaid Debit Card
If you choose not to have your Unemployment Insurance benefits directly deposited into your bank account, you will receive a Money Network/My Banking Direct prepaid debit card in the mail. You will receive the card in a plain, unmarked envelope between seven and ten days after you first apply for benefits. The card is valid for four years, so don't discard it before the expiration date.

If you reopen or file a new Unemployment Insurance claim within that period, your benefits will be loaded onto that same card.

You may also receive a prepaid debit card in the mail if you resume benefit certification after 28 days or more of inactivity. Claims become inactive when you don't certify your benefits for 28 days or more. If you reactivate your claim, you'll need to re-enter your direct deposit information. Otherwise, we'll automatically switch your payment method to the prepaid debit card.

Problem Clarification Questionnaire – Electronic Adjustment (E-Adjudication)
If the Department of Labor needs additional information about the claim you filed online, you may receive an email with a secure link that will take you to an online questionnaire. You must complete the requested information. Once they receive the information from you and, if necessary, from your employer, they will make a decision on your claim.

BC26B – Notice of Determination
After our fact-finding interview, you will receive a determination by mail indicating whether you are granted or denied benefits. It will also indicate the dates during which you are ineligible to receive benefits. If you disagree with the determination, appeal the decision.

AT2 – Notice of Hearing
If you or your employer file an appeal, you will receive a Notice of Hearing in the mail informing you of the date and time of the hearing and how to participate.
