Jersey Assistance for Community Care
JACC participants work with a care manager to develop a plan of care (POC) to best meet your needs. The care manager works with the participant to ensure that services are delivered as planned. Based on the results of a clinical assessment, the participant and their care manager collaboratively develop the POC. All JACC participants receive care management services. By providing a uniquely tailored support package for the individual, JACC delays or prevents nursing home placement.

A New Jersey resident age 60 or older who:
- requires nursing facility level of care, but wishes to stay home;
- You are not financially eligible for MLTSS (Managed Long Term Services and Supports) or other NJ FamilyCare programs;
- Does not participate or will no longer participate in the NJ Alzheimer's Adult Day Services Program or the State Respite Care Program;
- You have resources of $40,000 or less for an individual, or $60,000 for a couple;
- you are a US citizen or qualified permanent resident;
- has no alternative means of obtaining the necessary support; and
- live in the community, and not in assisted living, nursing home, or residential care facility
