The United States Citizenship and Immigration Services (USCIS) has issued new policy guidance aimed at strengthening the integrity of the Form N-648 review process, Medical Certification for Disability ExceptionsThis update marks a significant change in how applications for exceptions to the English and civics requirements are evaluated in the naturalization process.

With this new guidance, USCIS will focus greater attention on the accuracy of medical certifications and on identifying and preventing fraud.

Requirements to request the exception

Generally, applicants for naturalization must demonstrate proficiency in English and an understanding of the fundamental principles of U.S. history and government. However, those with a physical or developmental disability, or a mental disability that has lasted or is expected to last at least 12 months, may request an exception by filing Form N-648.

This form must be completed and certified by a licensed medical professional, who must clearly and thoroughly explain how the disability affects the applicant's ability to meet English and civics requirements. The mere presence of a disability is not sufficient to grant an exception.

Reviewing multiple forms

The new guidance also clarifies that submitting multiple Form N-648s at the same time may raise concerns about the credibility of the request and will result in additional review. This is implemented to prevent abuse of the system and ensure that exceptions are granted only to those who truly need them.

Immediate application

The updated policy is contained in Volume 12 of the USCIS Policy Manual and is effective immediately. It applies to all naturalization applications and related Form N-648s filed on or after June 13, 2025. This measure supports Executive Orders 14148 (Initial rescission of harmful executive orders and actions) and 14159 (Protecting the American people from invasion), reaffirming USCIS's commitment to a fair and transparent immigration system.
